I found no Sender Email address while checking your form. Please see the image below.
Setting up Sender Email could solve the problem I guess

This sometimes happens when the form embed codes are not executing properly into the website page.
Probably because you are embedding the source codes method and some codes in your page might block the elements.This might prevent the file upload field working correctly, so files are not correctly forwarded into our server. To prevent future issues, I would advise re-embedding your form using our iFrame Method: Getting-the-Form-iFrame-Code
Nevertheless, I've opened a ticket so our developers can see if they retrieve this file.

We apologize for this inconvenience, CreativeProsePublishing.
It appears that the file that was uploaded failed to upload correctly causing the file to get corrupted. This can be caused by faulty internet connection which may ruin the file upon transit. Unfortunately, this file can't be retrieved. What I can suggest is to contact the person who submitted the document and ask him to resubmit the file.
It would also be great if you can inform us as soon as the file has been submitted so we can identify further if there were other issues which may have caused the corruption of the file.
