    HTML format not appearing correctly in email submission

    I used the default HTML settings for the notification email. The test email appears correctly (like a table which is what I prefer) but when I try to submit a request via the Jot Form, I get this (just a snippet of the full email) see below:

    It seems like this form was generated using a JotForm Licensing. At any rate, let me suggest you to delete and rebuild the email alert. Then, submit a new form to see if it fixes the problem.
