    We are having issues with http://form.jotform.me/form/41261693699467 

    We wanted to change the last part to /contact, but are geting the message: 

    404 Not Found

    Hello,

    That was probably a temporary glitch - related to connectivity issues which are too broad to speculate on - please try refreshing the browser -

    If you experienced this error while trying to edit the form in the form buider, please log out and log back in  - that should restore the form builder

    If the issue persists - please let us know so that we can take a closer look at what might be causing this.
