How can I have a electronic signature placed.

I'm needing some kind of proof indicating a disclaimer has been read/agreed to.   I thought an electronic sigature would be the best way.  Once customers reads disclaimer and agrees, they would type in their name as a proof and it would email  the form signed to us.
