Problem with facebook APP - form requires people to log in to view

I installed in my facebook page 

The form works when i'm logged but when other users want to use if asking about the registration in jotform.

That is odd -

I tested the form on your facebook page using three different browsers - not logged in to either Jotform or Facebook - the result was the same:

I'm not sure what caused the behavior shown on your screenshot - but it seems you are in the page admin account which requires you to log in to Jotform to edit your form.

Please log out of both Facebook and Jotform and test the form  - prefarably on another browser - is the form loading on your end?
