the EMBED/create custom url spanner tool isn't allowing me to create http://www.jotform.com/OPB15. The embed tool says it is saved but it doesn't show up in the text box overhead.
Can you advise please.

Hello opb15,
Please be noted that "Custom URL" is construct in the following format:
http://JotForm_Domain/YOUR_USERNAME/CUSTOM_NAME
Example:
http://www.jotform.com/opb15/XXXXXXXX
Please be noted that http://www.jotform.com/OPB15 will not work. When you add a "Custom URL" text, the text will be added in the URL and then your form will be accessible.
As per the screenshot shared by you, I can see that you have added the complete from URL in the textbox. You need to simply add the name in the custom URL textbox.
