Fields missing from notification

    Can you tell me why when the data is emailed that first and alst name does not show up.  I have attached a picture of the email. An help would be appreciated.

    I created a copy of the form and did a test submission. The First and Last name were visible:

    Did you received the missing information in this test submission?

    Is this a repeated situation or did it happen only with one submission? Also is this data available in the submissions on your Jotform account?
