Clearing the form from previous data

my form seems to have cookie or cache, old info on form, we use the form for estimates but need a clear form every time it is accessed.  How can I set this up or add a "clear form" button?  

Please try removing the Autofill option on your form, this way the data is no longer saved for later usage.

You can do so by going to Preferences on the top toolbar of the Form Builder, then clicking on Advanced Settings then click on Edit Autofill Settings.

Then disable the Autofill Settings and click on Save

You can also add a Clear Form button by going to the properties settings of your submit button and enabling the "Reset Button" option listed there.
