Can you help me find out why I'm receiving data from 2 date fields? I only have one on my form.

    Hello,

I see what you mean - I could also locate only one date field - yet your form submissions hava two date submissions from the field "Current Date"

I will ask our developers to take a look and advice.

We shall update you on this trehad upon any progress.

    Thank you for your patience.

    FYI...
As I'm working with conditional date statements, it seems the first one listed isn't "mine".

    Hello,

It seems that there had been a glitch when you were creating your form which caused a certain field to show up on your submissions page and not on the actual form.  What I can suggest is to create a new form instead and delete your existing one.

I have manually created a copy of your form which you can clone on this link. (cloning your own form won't help since it will just copy the bug as well). Please let me know if you have already copied the form from my link so I can delete it on my list.

Another thing, I noticed on your 9th condition, it is using "Registration Information" field.  There are two "Registration Information" fields so I'm not sure which one is it that you actually used.

Please let us know if you have further questions or concerns.

    Thanks.
