    Hi,

    It looks like your conditions is causing the form to freeze. When I cloned your form and removed all the conditions, the form started to work normally.

    Did you made updates to your form recently? Did it work normally before? You can try to restore the form to its previous working version using Revision History. This might help.

    After selecting a form on My Forms page click "More" and then select "Revision History".

    If the above doesn't help, I think the best step you can take is to clone your form and remove all the conditions to it. Then start creating conditions 1 by 1 and then preview every 5 conditions you created so you can track what specific condition that is causing the problem. This could be due to a messed up codes pasted on the text elements or it might be something else.
