It is clear that according to HIPAA, a covered entity that uses JotForm must complete a business associate agreement with JotForm. In searching the forum, several others have asked about this but an answered has yet to be provided. Many of the requests which were posted in 2013 have answered by saying the question has been forwarded to higher ups. It seems that JotForm in other posts has stated that it is HIPAA compliant in that data is kept encrypted and secure, which a BAA is really only a document agreeing to continue to keep data safe and secure. It seems like many covered entities are interested in JotForm but need a completed BAA. Will JotForm sign a BAA, and if not please let us know so that we may pursue another service that will.
