forms not available

    Our website (comeone built it for us and we do not have a backup or access to the forms) used two forms 



    and your site says they are no longer available.  Can you tell us why the forms are no longer available and if there is a way to make them active again?

    It looks like the form for the patient history is still working. I see it posted here on the site:

    I'm still checking on the registration form and will update this ticket shortly.

    The registration form was disabled. To enable it, from our dashboard select the Preferences option, then toggle the status field to enabeld.

    I just enabled it and it appears on the registration page now,

    Forms can be disabled if our phishing filter flags them, or if a user disables it. Let us know if you have any questions or need further assistance.
