I cannot find my form

    I created a form that was embedded into a site. The form is still there, but now I cannot find it on my Jotform account. The las successful use of the form was 6/9/14 at 7:56 AM CDT. So far I have received 57 emails. 

     

    The emded code is: 

    <p><script type="text/javascript" src="//form.jotform.us/jsform/41495011634146"></script></p>

     

    When someone submits the form, they are not able to complete it. 

    Form over quota
    This form has passed it's allocated quota 
    and cannot be used at the moment. 

    Try contacting the owner of this form.

    Hi,

    Currently our guest accounts are limited to 20 submissions per month and you have exceeded that limit.  If you register for a free account that limit will be raised to 100 and our paid accounts allow for 1000+ submissions per month.

    I just upgraded to Premium and I still do not see my form in my account. The form still shows up in my site, but I still get the same message:

     

    Form over quota
    This form has passed it's allocated quota 
    and cannot be used at the moment. 

    Try contacting the owner of this form.
