This is a commercial application that we are having trouble with. When I just tried to access it, the license information was overlapping itself and I could not put in my information correctly. Had another not be able to access it last night at all.
Please update on what is going on...........

We are terribly sorry for this inconvenience. I tried to look for a JotForm form at drivetransland.com but couldn't find any.
Is your form embedded somewhere else? If so, may I ask for the URL of the exact webpage where it is embedded?

GO TO DRIVETRANSLAND.COM THEN GO TO DRIVERS, YOU WILL SEE AN APPLY NOW LINK ON THE LEFT AND THERE IT IS.
WE ARE WORKING ON AN EASIER ACCESS.
JUST SENT ANOTHER MESSAGE, HAD 2 MORE CALL IN SAYING IT IS STILL MESSING UP.

I tested the site version and the next button didn't work. I tested the standalone version and the next button was working but the required fields were over-reactive.
I cloned the form and it is working perfectly. I kindly ask you to clone the form (http://form.jotformpro.com/form/41635034319955?) and embed it again in the website:
