I'm trying to add a polling image slider widget.
I'm trying to add a polling image slider widget. Within each image, there should be a 4-5 multiple choice question.
How can this be accomplished?

I do not think that we can place the question fields directly on the slider images, but we might think about workarounds. Is that okay if we place the question fields below the slider field? Is there are any other requirements?
The questions/answers can be re-arranged on the page.
Will we have the ability to have immediate answers to the questions as soon as they're clicked?

We can not show answers within questions, but we can accomplish something similar with the Text fields and conditional logic on forms.
Please take a look at the following form:
I have used Image fields, Text Fields, Radio Buttons, Form Tabs widget and Page Breaks.
Then, added Show conditions.
