Hi,
I checked you website in Chrome, Firefox, and Safari, all of which seem to be working fine.
The left side being Firefox and the right being Chrome. I also checked your form and it seems to be setup correctly. We were having issues with submissions earlier and this may have been interfering with your form. If things are still not working they way you want them to, please let us know which browser you or your customers are using and we will be happy to test things further.
