    I WANT TO CREATE A MENU WHICH IS LIKE A VERTICAL COLUMN ALLIGN LEFT SIDE OF PAGE WITH MENU AND SUB MENUS.

    While JotForm is not optimized for such purposes as making menus, you can make use of the collapse form like the one that you see on this sample page - http://www.jotform.com/form/12292350328?
