Hi,

Our records indicate that you have several forms forms that set off our fishing detector.  Your forms appear to only ask for information such as email address' and not much more than that.  Due to your account containing multiple instances of this, it was disabled.  Please refrain from creating forms that do not have any other purpose other than collecting emails and/or passwords as they will continue to set off the phishing detector and automatically suspend the account
