Had just created a form that is now disabled. why? and how to fix it?

Hi,
Two of your forms set off our phishing detector and automatically suspended the account. Whenever someone asks for account details, such as Skype ID, along with email address and things of that sort, it raises the threat potential of the form. I checked your forms and they appear to be within our terms of use. I whitelisted your forms and reactivated your account. You should be able to use and access your forms once again.
