Recovering an old account

    Dear customer representative at JotForm,

    My name is Eugenia and my company hosts Craig Mueller's website http://www.yourlouisvillechiropractor.com/ They have a special offer form that they created but unfortunately can not get their log in credentials since an employee of his created it and no longer works at his office. On Wednesday the 18th I contacted you guys asking for his credentials and David from your team was very helpful and let me know that if we created a new account he would be able to move the forms to a new account.

    David mentioned that Dr. Craig Mueller's username for the old account is Annie.MuellerChiro@gmail.com-

    We have made a new account and the username is drcraigmueller and the email address to that account is eugenia.abalos@vortala.com Would you please help us and transfer the form over to this new account? The old one can be deleted after.

    Hi,

    I do remember answering your questions and I'm glad we could figure out where your forms were.  I do believe you are the original owners of the forms so I did go ahead and move them to your new account.  The account with the username "drcraigmueller" should now have all of your old forms, including their submissions.  Please let us know if there is anything else we can do for you and we will be happy to help.
