    Your CC Payment Form collects credit card information and this is a clear violation of JotFrm's Term os Use and the system automatically suspended your account. You can use any of our payment gateways should you need to process payments on your form. Kindly delete/modify the form to avoid this kind of trouble in the future.

    I have reactivated your account and you should be able to log in to your account now.
