Fields don't show in Update a Form Field Value

I like to configure conditions, so form fields are updated with values. However, in the conditions configurator, the fields are not shown (they disappear behind the configurator frame). See image. This happens in Chrome as well as IE browser. Can you fix this asap?

I did check your form but I am unable to replicate the issue you are having. The wizard seems to display the form field in drop down without any problem. Please check the screenshot below:

I would suggest you to clear your browser once and see if that solves your problem. The following guide should help you clear browser cache:  http://www.wikihow.com/Clear-Your-Browser's-Cache
