The account email is through firstname.lastname@example.org. She is happy to upgrade the account, but now the form is gone and she can't recover it.

Our records show the account associated with the email "email@example.com" has indeed upgraded to premium. I also see three forms associated with the account:
If these are not the forms you were looking for, please provide us with any other information you can such as the form URL, a webpage containing the form, the form ID number or anything of that sort and we will be happy to look into helping you recover the form.
