If I understand your message correctly, I think what you were trying to tell us was you are NOT able to logged in to your account?
You can reset your password using your email account.
Follow this step
1. Go to https://www.jotform.com/login
2. Click on the 'Forgot Password' option
3. Use your email email@example.com account to received the reset instructions
After you have successfully reset the password, you can now login using your sickat27 account/username
