Hi,

I checked your account and our records indicate your account upgrade went through.  I checked the form URL you provided also and the form is active.  The package your have purchased should allow for up to 1000 submissions and you are well below that limit.  Please let us know if you are still having an issue with your account and we will be happy to look into it further.
