Approval - Need more information

I need to create a workflow with approvals for workplace incidents that returns the form with a request for more information.

For example...I may get an incident report that a patient fell, but I need more information for the person who sent the incident.

I might also need to forward that incident report on to another employee who saw the patient fall but did not fill out the first incident.

Is that possible?
