Displaying multiple conditional fields

Is it possible to display multiple conditional fields at once?  Here's the scenario:

The form displays a select box with the title "How many issues did you encounter?"  The options are 1, 2, or 3.  If the user selects 2, then two separate input fields dynamically display, allowing the user to enter discriptions about each separate issue.  Similarly, if the user selects 3, then three separate input fields dynamically display.

    Possible?

    Hi E,

    Thank you for using JotForm.

With JotForm's Conditional Logic, you can accomplish that by adding individual conditions in your form. Your conditions should look something similar like this one.

You can check the form in action through http://www.jotform.com/form/12340424267. If you want, you can clone it and observe its conditions.

Hope this helps. If you have further issues and concerns, please feel free to contact us again.
