cant upload photos

MY FORM SUBMISSIONS work fine until I try to upload a photo. how can I fix this problem

I am able to submit to your form with an image uploaded to your file upload fields. Have you sorted this out? If not, please let us know if the form is embedded, give us the exact webpage where the form is embedded so we can further check.
