    Hi Scott Morris,

    Thank you for using JotForm.

Unfortunately, for security reasons, we cannot grant you access to the form. The best thing you can do is to create an account with JotForm, clone the form by obtaining its URL, copy the cloned form's code and replace the embedded code from your site. For the detailed instruction on how to clone a form with its URL, you can visit this page http://www.jotform.com/help/42-How-to-clone-an-existing-form-from-a-URL

But if you really need to access the form, I would suggest you to contact your previous web designer and ask to surrender its JotForm log-in information.

If you have any other problems, please feel free to contact us again.
