My form, https://www.jotform.com/jsform/10101248597 is all screwed up, and I really need help. There are bullet points showing up for now reason, and the conditions have all stopped working. What happened?

This appears to be a bug associated only with the secure version of your form. If you visit http://www.jotform.com/mariciamit/10101248597 you'll see what I mean.
I'll submit a ticket to see if we can get this issue resolved for you ASAP.

This problem is now fully resolved.
Yesterday we released a new major version for our deployment system. This will allow us to improve the speed in which we can develop new features and fix existing bugs. The new version runs the deployment processes and tests in parallel. So, we can do releases in half a minute as oppose to 5 minutes previously.
As a part of the new release there were changes on location of some of the CDN files. It turns out some CSS/JS files on SSL version had incorrect links. This is now fixed.

I am having the same issue. Everything worked fine from my office and when I got home the bullet points popped up.
Two of my forms are affected
They seem to be working fine out side of my Google site, perhaps it is a Google related issue...

Thank you for reporting, Justin. May I recommend that next time to please start a new topic so that we'll be able to submit a ticket to the development team for such cases like yours, since the issue discussed here is actually quite different compared to yours.
For the meantime, you can try using other iframe gadgets for your Google site to have the bullets fixed. You can read on to Setting the form background transparent on a Google site guide (at the lower part of the guide).
I also have created a bug report regarding your issue. We'll let you know as soon as we have updates.
