Upon checking your web page URL where you have embedded your form, I found that it is taking the responsive layout since you have added the "Mobile Responsive" widget. Accessing it from a normal system should not make any difference in look and feel but in case of your form it does.
I cloned your form in my account and when I accessed it with it's direct URL, it seems to work normally. To test the issue further, we would like to check your form directly. It seems you still have a guest account and that is the reason why I am unable to check your form. Please sign up for a free JotForm account using your email address "email@example.com" and the form will automatically move to your new account.
