I checked the only form we show associated with your account and it appears you have one empty email notification and one that does not have a sender email which will default to "email@example.com". The first image is the one that has no address to be sent to and does not get sent:
The second is not setup to come from a custom domain and defaults to being sent from "firstname.lastname@example.org":
I am not sure what could have caused this change. Here are our guides on how to set these up again just in case:
