Field label renamed, now conditions stopped working

I renamed the field label. It appears it dynamically changed in the Condition settings. However, after #3, with Add Family Member checked, #4 does not appear as expected.

I have cleared my JF cache as well as browser history/cache to no avail.

Please advise.

Perhaps you selected the wrong "Add Family" field on the 4th condition? Just a thought since this corrects everything after doing the following.

I did re-select the Add Family Member on the 4th condition and everything runs smoothly on my cloned form.
