Radio button to display different fields

    Hi,

    I'm creating a form that should allow the visitors of the site to ask me questions about services offered. And I'd like to give them a choice of the way how I will answer their questions - by E-mail or by phone.

    The first field should be 'Your name'. It's simple.

    The second field should be 'How do you want us to reply to you?' It has two radio buttons: 'by E-mail' and 'by phone'. By default, the 'by E-mail' option is selected and therefore third field is 'Your E-mail'. But, if the visitor selects 'by phone', the third field changes to 'Your phone' and 'Your E-mail' field is no more displayed.

    Is it possible to create such thing with JotForm?

    Hello AlexKotov,

    You can achieve your requirements by applying conditions for Show / Hide fields based on a selection.
