    Question about a Registration form for Vacation Bible School

    I've created a form for parents to register their children for Vacation Bible School (VBS). Here's the problem - some have more than 1 child to register (some have 4 or 5) & I'd like for them to not have to fill out an entire new form for each child.  A lot of the info will be the same for each child (parent name, address, phone #, etc.).  If I just add extra spaces on the original form, then new fields (columns) are created in the database.  I'd like them to be seperate entries in the database.  Any thoughts on how to do this (is it possible?).  Thank you!

     

    Addendum:  Just wondering, due to no responses thus far: Does my question not make sense OR is there no solution to this problem?  Thanks.

    Hi AmerDamer,

    Thank you for your message. I believe I understood your inquiry. You want your registration form to have the option to take more than one child per family (or per form submission) but you do not want to see all children in a single entry (or row) in your database. Instead, each child will have his/her own entry or own row in the database but with other general details such as parent's name, address, etc carried over this entry. 

    If my understanding is correct, then this is not directly possible with JotForm. However, a good, clean workaround is to download an Excel or CSV copy of the form submissions (I assume that your database can import data from XLS or CSV files). Then manually copy the first child's record or entry) in the database, then paste it into a new record (or row) and then changing the child's name accordingly.

    Hi.  I'm revisiting this thread.  Please see my original post to understand what I'm trying to do (although replace the word "column" with "row").  Is there a way to submit a form with just one child entered, but have the submit button linked to a new URL - one that takes the user back to the form, but with certain info filled in (i.e. the family info that will be common for all the children).  Then they can proceed to fill in child #2's info & submit that as a seperate entry.  Make sense?  Is this possible?

    I get the feeling like you want:

    1. All the children to have different form submissions

    2. The same parent information across different form submissions

    Am I right? Let me know if I got it wrong.

    Then yes this is possible:

     - You can accomplish this using the Thank You redirect URL.

    2. "..One that takes the user back to the form, but with certain info filled in.."

     - This can be accomplished by combining the feature linked above, with the redirect URL as the form's URL, but set to prepopulate:

    Currently working on a demostration, will update this thread as soon as it is complete.

    The only problem I see is that it appears I'd have to prepopulate the fields.  What I would want it to do is to draw the info for prepopulation from the data they just entered on their previous submission.

    About Pre-population:

    Its fairly technical. It requires stripping the fields of information, then including the stripped curly braces in the pre-population URL.

    After the URL is ready, you can place it in the Thank You Message.

    How does it work?

    Once you have filled it out and submitted it will take you to a page where you have options:

     

    1. Add another Child

    2. Take you back to a website page.

    3. All the children will be on different form submissions, but the forms might have the same Parents depending on the number of Children.

    Give it a run and let us know whether it's close to what you are looking for.

    Hi.  When I tried using the Field Prepopulator app, I was able to select the form I want to use it for, but then when it went to the next screen, none of my forms field show up on the screen.  Do I have to somehow designate those fields in order for them to show up on there?

    That is indeed unusual.

    Is this the same behavior when you try to use another browser?

    I'm using the Childrens Ministries Registration form.  When I'm in Google Chrome, no fields show up in the box.  I tried Internet Explorer.  When I tried to use the app, it asked me to login to Jotform (which I was already).  When I filled in my login info, that info just cleared out of the fields & didn't go anywhere.  Hmmm.  Not sure.  What do you suggest?

    Hi.  I would like to learn how to do this.  Would it be possible for me to create this custom URL.  If you just give me the pattern, so I know what to insert where, I could do it.  I will need to do it for one of my other forms also.

    Also, I got the Field Prepopulator app working finally :0).  If you tell me how to fill in the fields on there in order to draw the data from the previous submission, that'd be great.  (Not sure if that's how it works.) LOL

    The app is pretty useful when generating static names, like Dad/Mom's real names, but for this purpose, it requires a little tweaking, which is a lengthy process. 

    So what are all those {} braces about? They draw your data from the filled form and prepopulate it in the next.

    Yes, I will deleting the other Children Info fields.  I just didn't want to do that until I made sure this worked. ;0)

    Two questions for you?  Why does this form number work: 40283956276968    Where did it come from??

    Also, is there a way to jump back into the form at a certain page?  For example, if I want them to go right to the child info page when they click on the link.

    Sorry, that must have been the test form I was working with. 

    Just change the form number (found in your form's URL) to yours - they are similar. If the link has any issues, let us know.

    To Jump to the page that you want, use the conditional logic wizard:

    You can select the last field on the form that the parents would have filled to skip the page, for instance:

    Hi.  I'm back.  Some problems maybe you can help me figure out.  After my initial submission, I click on the link to go back to the form with prepopulated fields.  I'm having a problem with the data from {cellPhone} filling into the field {cellPhone293}.  I'm not sure why.  Then the field {cellPhone} is blank.  Also, another weird thing...on the prepopulated form...when the cursor just passes over the phone # fields, the prepopulated phone numbers disappear.

    I am sorry if I suggested this, but this thread has gotten too long already, and I am having difficulty tracking which form, and what is the issue was about. From what I had back read, it seems the original reported issue was resolved already. So, it will be best if create a new thread for the new reported issue.
