Unable to submit form. It always says Please wait..

This form is not receiving submissions. 

When submitting it hangs up on "Please Wait"

I'm sorry for the trouble caused to you.

I did test your form and I am able to replicate the issue you are having. You have added several "Text" field in your form which seems to have copy pasted formatting. I did edit all of them and now the form seems to be able to submitted successfully.
