I've gone through the ftp wizard several times and after I verify that the ftp information is correct, I click "next" but the folder's never show up. The icon just keeps spinning. I do get the green check mark and I've tried passive and not passive. I've also tried changing permissions. It never shows me the files. What gives?
Also... If I try putting in the path and clicking finish and/or just clicking finish I get this error (yet I know my credentials are correct)

It is possible that the upload path/folder that you have assigned for the FTP does not have the sufficient write permission also.
I have just test the form submission FTP integration , and I was able to successfully integrate just fine.
Check if the folder/path have the 'write permission'
Inform us if still not resolved.
