Form not integrate with Google Sheets.

For some reason my form will not integrate with Google Sheets. I have done this integration several times with other forms, but it doesn't appear to be working for this form.
Please advise,

Hello Kristi,

Thank you for contacting Jotform Support. I assume you're referring to Employment Application Form , it seems that your other form doesn't have issue integrating with Google Sheets and Google Drive, as shown in the screenshot below:
I cloned your form and tested it to see if I could replicate the issue, but everything was working properly. Check out the screencast below to see my results:
Maybe the issue resolved itself. That happens sometimes. For extra measure, I've cleared your forms caches. Can you try it again and see how it goes? If you run into the same problem again, let us know and we'll do some more testing to see what's going on.
