I would like to create a simple evaluation form, but the formatting options seem a bit rigid. Is there any way that I could include options 1-5 and "N/A" all on one line rather than on two? I would like clients to be able to fill out the form online, so it needs to look well assembled.
You will see in the picture that the radio butts for "1" and "N/A" have been pushed down to a second row. Increasing the number of columns does not fix this issue.
