I see that there are no forms currently in this account. However, I see a guest account registered with same email d. I went ahead to reproduce the error on this form. I tried to submit the form and I am able to see Thank you page which shows that the form is working fine.
This could also be an issue with browser's cache. I request you to clear the browser cache by following the steps provided in
