Error in Form

My account is "andyroney"

I have a form with radial button for selecting on a 20 question questionnaire.  It is required and this is selected.  This form has worked perfectly until the last few weeks.  Now even if you select an answer and click submit it says incomplete values:

Please can you see what may have changed and what is causing this error to occur now.  We use this form to get important submissions from parents and I need it up and running asap.

I have tested your stand alone form URL and submission was ok https://www.jotform.com/form/12501926183. There could be some conflict with the scripts that's on your Iweb.

If you have followed this guide https://www.jotform.com/help/12-Adding-Form-to-iWeb and form does not work correctly I suggest you try the iframe code and see if that works.

Please update us if this still does not work.
