I upgraded to premium service, received a transaction confirmation from PayPal. Why am I getting cancellation notices from you, told I've reached capacity and my forms will be removed?

I checked on this problem and found that you weren't logged in correctly at the time of purchase. So because of this it didn't associate the purchase with your account properly.
Your username should be where I've highlighted in red below, but as you can see it's undefined and I have manually upgraded you to fix it for you.
