I see that you are using password fields in the form which is not allowed and against to our terms of usage and hence the form is disabled. Our anti-phishing system is strict and scans every form to keep user's data safe and secure. We request you not to use any password related fields, accept sensitive information like credit cards, etc that are considered as phishing and throws the account to suspension.
