Revise form style from list to drop down menu list and/or dynamically adding records

    Hello,

    I'm asking for help or suggestions on how to improve this form from a list type form to dynamically adding records instead. http://form.jotform.us/form/33564942550154 Although I feel it would be better to use this form than our current method. (see pic I've added). I still feel it can be improved.

    I have hundreds of store names and addresses that I'd like to add to a drop down list. I know the drop down exists, and is easy to setup, the issue is that there has to be a picture uploaded with it. It's used for proof of product placement in each store. The current form we use (pic i've added here) can only upload one store address and pic at a time. This is a tedious task for our distributors who at times have to submit between 1 and 100 stores.

    The widget i'm interested in, and what I'm assuming might let happen, is that instead of having the list of stores, (i.e. URL i added above) the form would dynamically add records according to how many the user needed to upload. They could pick the store from a drop down list, upload a pic, then hit a + sign to create another record.

    The other form alternative would be to create multiple drop down lists with all of the stores listed and a pic upload button next to each one. If it has to come to that, I will do it, but the form would be much cleaner if the records could be added how I described above, if not, I'd have to create 100 drop down lists and upload buttons on the form.

    Hoping you can understand my explanations and needs.

    Hi,

    You could use something like our matrix dynamique widget:

    This would allow you to have a similar setup as the form you referenced and allow for users to add as many replicated field layouts as they would like. If this widget is does not accomplish what you are looking for, please let us know and we will be happy to see what else we can find.
