How to show multiple questions using Condition Statement

    Hello,

I am having trouble with the conditional formatting. I am trying to set up my form so that when a respondent chooses option x from a drop down, the non-applicable fields are hidden.

The question in my form is the "select only one option from the mechanism of injury list" near the end of my form.  For example, if the respondent chooses "falls, trips and slips of a person" from the dropdown, I want all the following fields to be hidden aside from the field for "Group 0 - Falls, trips and slips for a person" .

When setting up the conditional formatting, it only works when I select that first option of "falls, trips and slips of a person" and not for any other options.  Even though I have made the same conditions for the other options in the drop down (see uploaded screenshot).

Does anybody know what might be the issue here?

    Thanks,

    Nick

    Hi Nick,

Please review this user guide http://www.jotform.com/help/297-Show-or-Hide-multiple-fields-at-once-using-form-collapse-tool

It will be easier if you add collapse field in the group of fields you wanted to group. This way your conditional statements will be easier to manage.

AND by default you should always use SHOW Actions if you want to HIDE fields. If you observe how the condition statement works. If you use SHOW it will definitely HIDE the fields by default. (since its needs to be hidden before it can be shown)

I used your form to show you how to do this. See screens below

the final conditions is like this

Here you can try my test form http://www.jotform.me/form/42377460705456 and you can see it working as intended.
