Hello RZais,
Please be noted that it is currently not possible to connect directly to your MySQL from the form builder but there are ways to send submission data to your MySQL database.
Here are the available options:
1. You can enable the "Send Post Data" of your form preferences and then redirect users to your own thank you page where you can read the submission data and save it to your MySQL database.
