First of all, thank you for using JotForm.
If ever a form is deleted while currently embedded in a website, the appearance and some of the functionalities of the site can be affected depending on what embedding method has been utilized. If you're using the default embed with script tags, the part, where the form is placed, will display nothing. If, however, iFrame was used, an image like this one below will be shown.
But if you've inserted the form using the full source code, the form will still appear but the submissions that will be made can no longer be accessed unless it is housed on your own server.
