Is the 100 submissions and 10 SSLs a per month cap and if so, how are months calculated (e.g., by days or calendar months)? If not, what is the time period covered?

Yes that is the monthly cap for the Free Plan. It's reset timeframe is always on the first of the month. So if you exceed that limit then this means you either have to wait for the reset to occur or just upgrade your account.
We have various plan levels and they are all per-month based. But they can be purchased ahead of time for the long run which is a much better deal per year and biyearly.
Their timeframe is still used for an allotment on a month-to-month usage amount still, but the reset day is based on the date of purchase instead. Also, the same upgrade rules apply if any of the usage limits is hit.
