Why is my form not opening on iPad?

Hi - I'm trying to view my form on Google Chrome and Safari and the form is not opening. I'd like to have my clients input their information onto the form through my ipad and be able to submit it directly through there. 

Can you please tell us which form you are having this problem on or provide us with a link to it so we can test this further?
