I just used it and generated the URL with pre-populated date field , and it works OK in Chrome.

Yes, its work, but the date field is hidden. When i use the App, hidden fields are hidden!. When the field is hidden the form dosn't work (hidden field is blank) and if it is not hidden its work but when you hide the field is blank again

It is hidden because that's what the hidden fields are intended to do even when using the app as Jeanette had previously pointed out since that works.
I tested this on your form but it's showing up blank in both chrome and ie for me but I see you have tried to populate it as above if perhaps that might have been what you meant?
I was testing this further and when I dug into your form more I discovered you have some conditions tied to it that appear to be causing it to not be working right.
Even if you attach a populated parameter to it because it has a condition attached it would still be ignored since the condition would automatically override that unless the rules are met.
